Bikeability - Monitoring and Evaluation

This survey is for instructors to undertake on behalf of the children to understand the impact of the bikeability training. The first half of the survey is to be undertaken before the training and the second half of the survey is to be taken after the training. Once both sections are filled in, the survey should be submitted.
